I think this might be a bug in urxvt, it's not always the 4th window, it seems to be a different numbered window each time the number of windows upwards from the old effected window, changes.  I'm using tall, and I don't have any options set that could possibly make my xmonad configuration weird.  I'll try installing xmonad on another comp and seeing how it fares...
