[xmonad] Firefox `save as` dialog doesnot appear ?

It's does not appear (i.e. appears under other windows) because you swap it
down (class "firefox") at your manageHook.
You may want to use ManageHelpers composeOne instead of composeAll, so when
a window is matched (in this case, to float) the following rules won't be
evaluated (in this case, swapDown).
