Comment:

 I think this bug is fixed in 7.2.1 and later, but it's not clear what it
 was and the bug has been impossible to reproduce.  There have been
 multiple reports, e.g. #5588.  There's also #4245 but that appears to be a
 different bug with the same symptoms.
