Comment:

 I'm going to go ahead and close this.

 I was never able to reproduce it reliably, but a couple weeks ago I found
 a fishy bad array read in c++ and since I fixed that I haven't seen it
 again.  So chances are good that was the problem.

 Sorry about the false alarm!
