 On iOS configure gives us...

 #define HAVE_SETITIMER
 }}}

 so setitimer is our only option. I re-tested the existing Itimer.c and it
 doesn't work (it doesn't fire).

 So I've added a patch here to give a pthread-based implementation that
 does work on iOS.
