EPA: Remove AddEpAnn Commit 8/final

EPA: Remove AddEpAnn from AnnList

EPA: Remove AddEpAnn from GrhsAnn

This is the last actual use

EPA: Remove NameAdornment from NameAnn

Also rework AnnContext to use EpToken, and AnnParen

EPA: Remove AddEpAnn.  Final removal

There are now none left, except for in a large note/comment in
PostProcess, describing the historical transition to the
disambiguation infrastructure
