  Tidy up long-standing usability annoyances re: the use
  of scrollbars in the command window:
  
     - thumbs are now proportional to number of rows of output
       displayable in the view. Resized automatically as more output
       gets added.
     - cursor/caret is only displayed if it's inside the view.
     - if the caret is outside the view, use Enter (and other
       'control' keys) to scroll the caret into view.
