 I have updated the GhcAstAnnotations page to reflect the current design,
 as well as to add a section outlining an approach to comments, which is
 https://ghc.haskell.org/trac/ghc/wiki/GhcAstAnnotations#comments

 The comments have been added in Phab:D297, causing the file
 `ApiAnnotations` to be moved into `Lexer.x`
